Selon un communiqu\u00e9 publi\u00e9 \u00e0 l\u2019occasion de la 13e conf\u00e9rence de la Soci\u00e9t\u00e9 internationale du sida (IAS 2025), qui se tient \u00e0 Kigali, au Rwanda, le l\u00e9nacapavir offre une protection quasi totale contre le VIH aux personnes expos\u00e9es \u00e0 un risque \u00e9lev\u00e9.<\/p>\n<p>L\u2019OMS estime que cette avanc\u00e9e pourrait \u201c<em>remodeler la riposte mondiale au VIH<\/em>\u201d, en particulier dans les groupes confront\u00e9s \u00e0 des obstacles structurels : difficult\u00e9s d\u2019observance, stigmatisation, ou encore acc\u00e8s limit\u00e9 aux soins. \u201cA<em>lors qu\u2019un vaccin contre le VIH reste hors de port\u00e9e, le l\u00e9nacapavir est ce qui se fait de mieux : un antir\u00e9troviral \u00e0 action prolong\u00e9e dont les essais ont montr\u00e9 qu\u2019il permettait de pr\u00e9venir la quasi-totalit\u00e9 des infections \u00e0 VIH chez les personnes \u00e0 risque<\/em>\u201d, a d\u00e9clar\u00e9 le Dr Tedros Adhanom Ghebreyesus, Directeur g\u00e9n\u00e9ral de l\u2019OMS.<\/p>\n<p>Cette recommandation intervient alors que la pr\u00e9vention du VIH stagne. En 2024, 1,3 million de nouvelles infections ont \u00e9t\u00e9 enregistr\u00e9es dans le monde, avec un impact disproportionn\u00e9 sur les populations vuln\u00e9rables : travailleurs du sexe, hommes ayant des rapports sexuels avec des hommes, personnes transgenres, consommateurs de drogues injectables, d\u00e9tenus et enfants.<\/p>\n<p>Outre le l\u00e9nacapavir, l\u2019arsenal pr\u00e9ventif soutenu par l\u2019OMS comprend \u00e9galement les comprim\u00e9s oraux quotidiens, le cabot\u00e9gravir injectable (tous les deux mois) et l\u2019anneau vaginal de dapivirine. Dans un contexte de r\u00e9duction significative des financements mondiaux \u2013 notamment du PEPFAR, principal programme am\u00e9ricain de lutte contre le VIH lanc\u00e9 en 2003 \u2013 l\u2019OMS a publi\u00e9 de nouvelles orientations op\u00e9rationnelles visant \u00e0 maintenir l\u2019acc\u00e8s aux services essentiels de pr\u00e9vention et de traitement.<\/p>\n<p>\u201c<em>Nous disposons des outils et des connaissances n\u00e9cessaires pour mettre fin au sida. Ce qu\u2019il nous faut maintenant, c\u2019est une mise en oeuvre audacieuse de ces recommandations, fond\u00e9e sur l\u2019\u00e9quit\u00e9 et aliment\u00e9e par les communaut\u00e9s<\/em>\u201d, a affirm\u00e9 la Dre Meg Doherty, Directrice des Programmes mondiaux de lutte contre le VIH \u00e0 l\u2019OMS.<\/p>\n<p>\u00c0 la fin de l\u2019ann\u00e9e 2024, 40,8 millions de personnes vivaient avec le VIH dans le monde, dont 65 % en Afrique subsaharienne, selon l\u2019OMS. Cette m\u00eame ann\u00e9e, environ 630 000 personnes sont mortes de maladies li\u00e9es au VIH et 120 000 enfants figurent parmi les 1,3 million de nouvelles infections. Malgr\u00e9 ce bilan pr\u00e9occupant, l\u2019acc\u00e8s aux traitements antir\u00e9troviraux progresse.<\/p>\n<p>En 2024, 31,6 millions de personnes b\u00e9n\u00e9ficiaient d\u2019un traitement, contre 30,3 millions en 2023.
